Five people went into the water yesterday and didn't come back.

Five Italian tourists, diving off Alimathaa island in the Maldives.

And the reports are still thin, but that number is extraordinary.

Octavio ES

It's a number that doesn't make sense at first glance.

One diver dying in an accident, sadly, happens.

But five people from the same group, in the same place and on the same day, that's no longer bad luck.

Eso es otra cosa.

That's something else.

Fletcher EN

Right, and that's exactly what pulled me into this story.

Because the natural reaction is to file it under 'tragic accident' and move on.

But the moment you ask what could kill five trained recreational divers simultaneously, you end up somewhere much more interesting.

Octavio ES

I've dived a little, I'm no expert, but enough to know that the sea doesn't forgive certain mistakes.

And the Maldives has currents that can be treacherous, especially in the southern atolls, where Vaavu is.

Fletcher EN

Before we get into the possible causes, can you set the scene?

Because listeners who haven't been to the Maldives might picture some calm lagoon.

Vaavu Atoll is not that.

Octavio ES

The Maldives is actually an archipelago of about 1,200 islets spread over almost 900 kilometers in the Indian Ocean.

Vaavu Atoll is south of Malé, the capital, and diving there is famous precisely because the underwater terrain is complex: there are deep channels between the atolls where currents are very powerful, especially during tidal changes.

Fletcher EN

And Alimathaa specifically has a reef that's well known in the diving community.

It's the kind of site that attracts experienced divers, not beginners.

Octavio ES

Exactly, and that's what makes the story so disturbing.

We're not talking about inexperienced tourists who got in the water not knowing what they were doing.

From what is known, they were adult Italian divers who, presumably, had some level of certification.

Y aun así, cinco de ellos muertos.

And yet, five of them dead.

Fletcher EN

So walk me through the main things that can kill a diver.

Because I think most people have a vague notion of 'the bends' and that's about it.

Octavio ES

Hay varias categorías.

There are several categories.

Decompression sickness, which is what people know as 'decompression syndrome' or colloquially 'diver's embolism', occurs when a diver ascends too quickly from depth and the nitrogen dissolved in the blood forms bubbles in the tissues.

Puede ser fatal, o puede simplemente paralizarte.

It can be fatal, or it can simply paralyze you.

Fletcher EN

The bends.

Named because the pain makes you bend over double.

I knew a photographer who got it off the coast of Borneo and he described the joint pain as the worst of his life.

Octavio ES

But there's another cause that is perhaps less well known and that statistically kills more divers than people realize: hypoxic syncope, or what in English is called 'shallow water blackout'.

It occurs when someone hyperventilates before submerging to stay underwater longer, and this reduces the level of carbon dioxide in the blood, which is the signal that tells the brain you need to breathe.

You can then lose consciousness without any prior warning.

Fletcher EN

Without any warning.

You don't feel yourself fading.

That's terrifying.

Octavio ES

And if five people from the same group were doing breath-hold dives, that is, without breathing equipment, or if something in their autonomous diving equipment failed systematically, it could explain multiple deaths.

Aunque también hay que mencionar las corrientes.

Although we must also mention the currents.

A strong current can separate a group of divers from their support boat in a matter of minutes.

Fletcher EN

Which brings up something that most people don't think about when they book a dive holiday: where do you go if something goes wrong?

The Maldives is not exactly flush with trauma centers.

Octavio ES

This is the part of the story that worries me most from a public health point of view.

The Maldives receives more than a million and a half tourists a year, many of them divers, and yet the medical infrastructure outside of Malé is extremely limited.

The only national reference hospital is in the capital.

In remote atolls, at best there is a small health center.

Fletcher EN

And Vaavu Atoll is roughly a hundred kilometers south of Malé.

By boat, in an emergency, that's not a trivial journey.

Octavio ES

There are seaplanes, there are speedboat transfers, but the emergency medical logistics of an archipelago of twelve hundred islands is a problem without an easy solution.

And for decompression sickness specifically, what you urgently need is a hyperbaric chamber, which recompresses the patient to reabsorb the nitrogen bubbles.

In the Maldives there are very few of those chambers, and not all of them work correctly.

Fletcher EN

Walk me back in history for a moment.

When did we actually figure out what was happening to people who came up too fast?

Because this wasn't always understood.

Octavio ES

The first documented description of what we now call decompression sickness comes from the mid-nineteenth century, when pressurized air caissons began to be used for major engineering works, like the pillars of bridges over deep rivers.

Workers who emerged from those pressurized caissons sometimes collapsed.

The doctors of the time called it 'caisson disease'.

Fletcher EN

The Brooklyn Bridge.

Workers building the foundations in the 1870s were getting 'the bends' and nobody had a name for it yet.

Octavio ES

Exactamente.

Exactly.

Washington Roebling, the chief engineer who oversaw the construction of the Brooklyn Bridge, was partially paralyzed by decompression sickness.

His wife, Emily Roebling, had to learn civil engineering to oversee the work in his place.

It's one of those cases where a poorly understood disease changed the course of a great story.

Fletcher EN

I did not know that.

The Brooklyn Bridge was partly completed by a woman learning engineering on the fly because her husband had the bends.

History is genuinely strange sometimes.

Octavio ES

And the physics behind the disease took decades to understand correctly.

It was John Scott Haldane, a Scottish physiologist, who in the early twentieth century developed the first decompression tables for the British Royal Navy, establishing that divers should make stops at certain depths during ascent to give nitrogen time to leave the blood safely.

Those tables were the basis for everything that came after.

Fletcher EN

And now dive computers do all of that calculation in real time.

Every recreational diver wearing a dive computer on their wrist has a direct line back to Haldane's 1908 paper.

Octavio ES

But the dive computer doesn't save you if the current sweeps you away, or if you suffer a hypoxic blackout, or if you have an undiagnosed heart condition that physical exertion at depth triggers.

Because it must be said: the heart is another important factor.

Many diving accidents, more than are acknowledged, have underlying cardiac causes.

Fletcher EN

Which raises something I find genuinely troubling about recreational diving as an industry.

The certification process, at least for basic open-water diving, is not especially rigorous about medical screening.

Octavio ES

You're right, and it's a debate that has existed within hyperbaric medicine for some time.

In many countries, to obtain a basic diving certification, it's enough to fill in a medical self-exclusion questionnaire.

That is, you declare yourself healthy.

No hay ningún médico que te examine.

No doctor examines you.

And in people over forty, which is a very common profile among European diving tourists, the cardiac risk is real.

Fletcher EN

Let's go back to the Maldives specifically for a moment.

Because there's a tension here that's almost economic.

The entire national economy runs on tourism.

Diving tourism is a massive part of that.

There's a real incentive not to over-regulate.

Octavio ES

It's the classic dilemma of countries whose economic survival depends on an industry that carries risks.

If the Maldives tightens diving safety regulations too much, tour operators complain, tourists go to Thailand or Indonesia, and GDP falls.

But if they don't regulate enough, things happen like what happened yesterday.

Fletcher EN

Indonesia actually had a similar case a few years back.

A group of divers got caught in a channel current off Nusa Penida and several were lost at sea for hours.

It exposed exactly this problem: incredible diving, minimal safety infrastructure.

Octavio ES

And the problem is not only in developing countries.

There are diving areas in the Mediterranean, in Spain, in France, in Italy, where oversight leaves quite a lot to be desired.

What distinguishes the best diving operators in the world is not necessarily the country where they operate, but their internal protocols: the ratio of instructors to divers, equipment checks, meteorological and current checks before each dive.

Fletcher EN

And when you have five dead, the investigation will eventually come down to exactly those questions.

Was the current checked?

What was the visibility?

What was the ratio of guides to divers?

Were the dive computers functioning?

And critically, how long did it take for any medical response to reach them?

Octavio ES

And that, the response time, is probably the most decisive factor in any diving emergency.

Serious decompression sickness, if not treated within a few hours, can cause permanent neurological damage or death.

If you're in an atoll a hundred kilometers from a hyperbaric chamber, those hours count enormously.

Fletcher EN

There's a parallel here to something I've seen in conflict zones: the 'golden hour' concept.

Get a trauma patient to surgery within an hour and survival rates jump dramatically.

The same logic applies to diving emergencies, and remote paradise destinations systematically fail that test.

Octavio ES

There are international organizations, like Divers Alert Network, DAN, that work exactly on this: mapping where hyperbaric chambers are in the world, which ones work, what the evacuation time is from each popular diving spot.

And the data is, in many cases, quite worrying.

Fletcher EN

The Maldives is on that map with very limited coverage.

I checked before we recorded today.

Outside of Malé, the options are sparse.

Octavio ES

Which leads to an uncomfortable question that tour operators sometimes don't want asked: should diving operators be required to inform their clients, clearly and before signing anything, how long it would take for emergency medical care to reach the place where they are going to dive?

Not information buried in the small print of the contract.

Clear, explicit information, like when the pilot tells you where the emergency exits are.

Fletcher EN

That's a standard I think most people would consider reasonable, and yet I'd bet it doesn't exist in most dive tourism markets.

Not as a mandatory requirement.

Octavio ES

No existe.

It doesn't exist.

Y mientras no exista, esto seguirá pasando.

And while it doesn't exist, this will keep happening.

Five Italians who went on holiday to one of the most beautiful places in the world and didn't come back.

Eso no es solo una tragedia personal.

That's not just a personal tragedy.

Es también un fallo sistémico.

It's also a systemic failure.

Fletcher EN

Well said.

And the investigation, when it comes, will tell us more.

But I suspect the findings will be familiar to anyone who's ever looked at adventure tourism safety: the danger is known, the protocols are inadequate, and the financial incentives all point in the wrong direction.

Octavio ES

Hey, there's something you said earlier I want to pick back up.

You talked about the 'ratio of instructors to divers' and used the word 'ratio' in English.

In Spanish we also say 'ratio', but some people say 'tasa' or even 'proporción'.

What catches my attention is how certain technical words float between languages.

Fletcher EN

I was actually wondering about that.

You used 'ratio' yourself earlier.

But I noticed you also said 'tasa' at one point when we were talking about survival rates.

Is there a real difference between 'ratio', 'tasa', and 'proporción' in Spanish, or are they just interchangeable?

Octavio ES

No son exactamente lo mismo.

They're not exactly the same.

'Tasa' generally implies a measurement in relation to a population or a specific time.

You say 'tasa de mortalidad' or 'tasa de supervivencia', because you're measuring something against a universe.

'Proporción' is more general, a part over a whole.

And 'ratio' is the foreign borrowing, it comes from Latin but arrived in modern Spanish through scientific English, and it's used mostly in technical contexts.

Fletcher EN

So if I'm talking about deaths per thousand divers, that's a 'tasa'.

If I'm talking about one guide for every four divers in the water, that's closer to 'proporción' or 'ratio'.

Octavio ES

Exacto.

Exactly.

And if you ever want to sound like someone who knows what they're talking about, use 'tasa' when talking about health statistics and save 'ratio' for engineers and accountants.

Although if you mix the two up at a dinner in Madrid, the most likely outcome is they understand you anyway.

Not the kind of mistake that ends up as a family anecdote.

Fletcher EN

Unlike certain other errors involving the word 'embarazado' which I will never live down.

Right.

'Tasa' for health statistics.

I'll write that on my hand before the next Madrid family dinner.
