Fletcher EN

Two families got a phone call this week that no parent can prepare for.

Their child had gone to university in Kent, in the south of England, and died of bacterial meningitis.

Octavio ES

Exactamente.

Exactly.

There has been a meningitis B outbreak at the University of Kent, and UK health authorities have responded by launching an emergency vaccination program.

Dos muertos.

Two deaths.

Not statistics, but two people who were, weeks earlier, choosing courses or buying bedding for their dorm.

Fletcher EN

And Northern Ireland has already announced it's extending the offer to all students up to age twenty-five.

That tells you something about how seriously they're taking this.

Octavio ES

Yes, and to understand why that matters, you first have to understand what this disease is.

Meningitis B is a bacterial infection that inflames the membranes surrounding the brain and spinal cord.

It's not the only form of meningitis, but it's the most common in the UK and the most deadly among young adults.

Fletcher EN

The thing that makes meningitis so terrifying, and I've known people who've lost children to it, is the speed.

You can be fine on Friday and dead by Sunday.

Octavio ES

Eso es literalmente posible.

That's literally possible.

The bacterium Neisseria meningitidis can kill within twenty-four hours of the first symptoms.

And the problem is that at the start it resembles a bad flu: fever, headache, stiff neck.

By the time the septicaemia appears, the rash that doesn't fade when you press it, it may already be too late.

Fletcher EN

The glass test.

I remember a public health campaign in the nineties where they told parents: press a glass against the rash, if it doesn't fade, call an ambulance immediately.

Octavio ES

Esa campaña salvó vidas.

That campaign saved lives.

Y sigue siendo válida hoy.

And it's still valid today.

But there's something I find crucial about this outbreak: it's no accident that it happened on a university campus.

University students are, along with babies under one year old, the highest-risk group for meningitis B.

And that's not a minor biological coincidence.

Fletcher EN

Why university students specifically?

I'd have assumed the very young and the very old, the typical vulnerable groups.

Octavio ES

It's a combination of factors that rarely all occur together.

Primero: las residencias universitarias.

First: university residences.

Hundreds of young people sleeping, eating, and socializing in closed spaces.

Segundo: el comportamiento.

Second: behavior.

Sharing glasses, kissing, smoking, long nights that weaken the immune system.

And third, perhaps the least intuitive: many of these young people are asymptomatic carriers of the bacterium without knowing it.

Fletcher EN

Carriers.

So they carry the bacteria in their throat, pass it on to someone else who has no immunity, and that person gets sick while they never do.

Octavio ES

Exactamente.

Exactly.

In the general population, around ten percent of people carry Neisseria meningitidis without showing any symptoms.

In university residences during the first months of term, that percentage can spike to forty percent.

And that's where the dynamics become dangerous.

Fletcher EN

Forty percent.

That's nearly half the people in a given corridor potentially carrying this thing.

And the one person who hasn't built up any tolerance pays the price.

Octavio ES

And here is the part I find truly outrageous about this story, Fletcher.

Existe una vacuna.

There is a vaccine.

The MenB vaccine, marketed as Bexsero, has been approved in the UK since 2013.

But for years, it wasn't on the systematic vaccination schedule for teenagers or university students.

Solo para bebés.

Only for babies.

Fletcher EN

Wait, back up.

They had a vaccine, it was approved, and they weren't giving it to the most at-risk group outside of infants.

What's the logic there?

Octavio ES

La lógica era económica, no médica.

The logic was economic, not medical.

The body that decides which vaccines enter the NHS schedule, the Joint Committee on Vaccination and Immunisation, took years to approve it for babies and much longer to consider it for teenagers.

El argumento siempre fue el coste-beneficio.

The argument was always cost-benefit.

La enfermedad es rara.

The disease is rare.

No justifica, decían, vacunar a millones de adolescentes.

It doesn't justify, they said, vaccinating millions of teenagers.

Fletcher EN

Tell that to the families of the two students who just died in Kent.

Rare doesn't mean zero.

Octavio ES

That's exactly what the families who fought for years to change that policy said.

There was a very intense campaign, led largely by parents who had lost children, to get the MenB vaccine onto the NHS schedule for babies.

Lo lograron en 2015.

They achieved it in 2015.

Pero para los universitarios, la batalla continúa.

But for university students, the fight continues.

Fletcher EN

Twelve years between the vaccine being approved and it becoming widely available to the group that arguably needs it most.

That's a long time to wait if you happen to be in the wrong corridor at the wrong university.

Octavio ES

And what has triggered this vaccination campaign in Kent is not that the underlying policy has changed, but that people have died in a specific outbreak.

Es una respuesta reactiva, no preventiva.

It's a reactive response, not a preventive one.

Action is taken after the tragedy, not before.

Eso es lo que habría que cuestionar.

That is what should be questioned.

Fletcher EN

There's something depressingly familiar about that pattern.

I covered enough public health crises to know: the political will to vaccinate tends to arrive exactly one outbreak too late.

Octavio ES

Completamente de acuerdo.

Completely agreed.

And there's a historical context worth bearing in mind.

The UK had, in the late nineties, a particularly devastating meningitis C epidemic among teenagers.

The response was a mass vaccination campaign that reduced cases by more than ninety-nine percent within a few years.

Ese precedente existe.

That precedent exists.

El país sabe cómo hacerlo.

The country knows how to do it.

Fletcher EN

Ninety-nine percent reduction.

That's one of those public health numbers that should be printed on a wall somewhere.

It worked.

The model works.

So why is MenB being treated differently?

Octavio ES

Varias razones.

Several reasons.

The first is epidemiological: serogroup C was much more prevalent at that time than serogroup B is now.

B, although it dominates current cases in the UK, causes fewer total cases in absolute terms.

The second reason is economic: the Bexsero vaccine is expensive.

And the third, perhaps the hardest to articulate, is political: babies generate more media sympathy than university students.

Fletcher EN

That last point is uglier than it sounds.

A sick infant is a front-page photograph.

A nineteen-year-old who dies of meningitis on a Friday night is a paragraph on page seven.

The optics drive the policy.

Octavio ES

Hay algo de eso, sí.

There is something to that, yes.

Though it should also be said that the NHS is under enormous budgetary pressure, and that decisions about which vaccines to include in the schedule involve real tradeoffs between limited resources.

No es solo cinismo político.

It's not only political cynicism.

Es también una tragedia del sistema.

It's also a tragedy of the system.

Fletcher EN

Fair.

And to be clear, the UK has one of the most comprehensive childhood vaccination programs in the world.

This isn't a story about a country that doesn't believe in vaccines.

Octavio ES

No, en absoluto.

No, not at all.

It's a story about where you draw the line.

And about what happens when the line is drawn in the wrong place and two families receive the call you mentioned at the start.

The vaccination program that has now been launched in Kent is an emergency program.

The vaccine is being offered to all students on campus.

But it is local, reactive, and it does not change national policy.

Fletcher EN

Northern Ireland moving to extend it to all students up to twenty-five, though, that's something different.

That's a devolved health administration making a proactive call.

Whether the rest of the UK follows is the question.

Octavio ES

And it is a question that will generate real political pressure.

When one region offers something the nation does not, the inequality becomes visible.

Why does a student in Belfast receive the vaccine and one in Manchester does not?

Esa pregunta va a resonar.

That question is going to resonate.

Fletcher EN

This also raises a question I keep coming back to: what happens in countries that don't have the NHS model, where vaccination coverage is patchier and depends more on what a family can afford?

Octavio ES

Es una pregunta muy pertinente.

It's a very pertinent question.

In Spain, for example, the MenB vaccine only entered the state-funded systematic schedule a few years ago, and the incorporation was gradual, region by region.

There were families who paid hundreds of euros to vaccinate their children because the administration didn't cover it.

A vaccine that in theory exists for everyone, in practice only those who could afford it had it.

Fletcher EN

Which means in practice it was a vaccine for wealthy families.

For years.

A preventable death that correlates with income.

That's not a medical problem, that's a justice problem.

Octavio ES

Exacto.

Exactly.

And that pattern repeats itself in many countries.

The pharmaceutical innovation arrives, but universal coverage takes time.

In that interval, which can be years or decades, protection is determined by the parents' bank account, not by epidemiology.

It is one of the most brutal contradictions of modern health systems.

Fletcher EN

One more layer to this I want to push on: the speed of outbreak response.

When this happened in Kent, how quickly did UK health authorities actually move?

Octavio ES

The response has been quick in terms of what usually happens with this kind of outbreak.

The cluster was identified, the serogroup was confirmed, the vaccination program on campus was launched.

But one has to be honest: the speed of the emergency response does not compensate for the slowness of prevention.

Acting within forty-eight hours of a death is not the same as having vaccinated that person three years earlier.

Fletcher EN

Right.

And the students being offered the vaccine now, after the outbreak, are getting it reactively.

The two who died didn't get to be reactive.

They were the event.

Octavio ES

Hey, before we wrap up, I want to come back to something you said earlier without commenting on it: that meningitis can kill over a weekend.

That made me think about an expression I used earlier in this conversation that listeners may not have fully caught: I said that authorities acted 'a raíz de' the outbreak.

Fletcher EN

A raíz de.

I know raíz means root, like the root of a tree, but I've never been totally sure how it works as a connector.

It's not quite because of, it's not quite following, it's something more specific than either.

Octavio ES

Tienes buen instinto.

You have good instincts.

'A raíz de' implies that something arises directly from a preceding event, like a root sprouting from the ground.

It has a causal but also temporal connotation: first X happened, and from that Y was born.

The first suggests the action grew organically from that moment.

The second is more mechanical.

El segundo es más mecánico.

Fletcher EN

So it carries a sense of organic consequence.

Like, the thing that happened didn't just cause a reaction, it was the soil in which the reaction grew.

That's actually a richer image than anything English offers me in that slot.

Octavio ES

Sometimes Spanish does have more nuance, Fletcher.

No siempre, pero a veces.

Not always, but sometimes.

Remember that next time you try to speak with my mother.

Fletcher EN

Your mother and I have an understanding, Octavio.

She thinks I'm charmingly incompetent and I think she's entirely right.
